To address the root causes of patients’ repeat visits to the ED more effectively and to reduce hospital admissions, physicians at Bon Secours Mercy Health (BSMH) in Ohio, South Carolina, and Virginia are given detailed recommendations for how to treat individual patients. Medical experts work together to create management plans that outline a patient’s medical history and the best care approach to meet that patient’s unique needs. For patients who frequently visit the ED, management plans led to a 64% decrease in ED visits, a 64% decrease in hospital admissions, a 72% decrease in opioids administered, and a more than 60% decrease in X-ray, CT, and MRI studies and associated radiation exposure.
